Welcome to our Online Patient Center! Here you can get information about what to expect during your visit with one of our optometrists as well as important forms you can complete before your appointment to save time.

We want your visit to be as stress-free as possible. Our staff will do everything we can to make sure your experience is pleasant and convenient. If you have any questions about these forms, please don’t hesitate to contact us.

Appointments for an eye examination are required. We recommend an appointment for all other services, including eyewear pickup and adjustments. Please call with any emergency such as pain, flashes, floaters, infection, or foreign bodies. We will discuss your issue and make an appointment within the recommended time based on your symptoms.

We confirm appointments by automated email, phone calls, and text. If you are unable to attend your appointment, please cancel as early as possible to allow time for someone else to be seen in your place.

Please bring your most recent pair of glasses and/or contact lenses to your appointment. Also bring a current list of medications and allergies. We ask that new patients complete their paperwork online ahead of their appointment to save you time in our office.

McWilliams Vision Care believes in providing the best value for you and your family. We have products and pricing that aims to accommodate all budgets.

Our office accepts cash, check, and all major debit and credit cards, as well as Care Credit.

All of the above plans can be used for all your eye care and eyewear needs, including eyeglasses, contact lenses, routine office visits and medical office visits.

If you do not see your insurance provider listed, please contact our office for more information. Accepted insurance policies include: Medicare, Medicaid, Healthy Indiana Plan, Anthem, Aetna, Cigna, Encore, Humana, Indiana Roofer’s, NECA, Sagamore, and United Healthcare.
